        <title id="abstract-title-d516da644aae4b3bb805daecf73d6695">Abstract</title>
        <p id="paragraph-e68493d9c3ef43ec9214d679e1a506a9">Endometriosis is one of the most common benign gynaecological diseases characterized by the implantation and growth of viable endometrial tissue outside the uterine cavity. The symptoms of endometriosis can be correlated to many of the <italic id="e-cf91d0be97af">Yoni vyapads</italic> described in our classics. In the present case, a 37-year-old female (Swedish national) reported chief complaints of severe lower abdominal and pelvic pain and heavy bleeding during menstruation from the past 7 years, especially one day prior to menstruation and stops 1-2 days after the onset of bleeding. She was advised for a USG scan which showed endometriosis, cyst in the left ovary. <italic id="e-cecf5ad5cc36">Sodhana chikitsa</italic> was followed, and the whole treatment period spanned for a total of 42 days. The patient was advised to follow up after 6 months. After the treatment, there was a significant improvement in the patient’s conditions, and in turn, the patient also expressed greater satisfaction with the result.</p>

      <title id="title-2f429938d1024813b97ffc784de9d300">Introduction</title>
      <p id="paragraph-6374b256d7de4021b029f09e82a83fb2">Endometriosis is the most common problem encountered in Gynaecology. This is a hormone-dependent gynecologic disease, with clinical manifestations of adhesion and progressive growth of endometrial cells in organs other than the uterus. <sup id="superscript-24010dfe16bf4c6da13a809dec094839"><xref id="xref-3c2a7cf31b5c4283a5307898641a7b0f" rid="R133176324214623" ref-type="bibr">1</xref></sup> This disease affects a large portion of the population. Half of the patients with endometriosis display clinical symptoms of pelvic pain and infertility, thereby reducing the quality of life of women. <sup id="superscript-241aac5df1cf4e9d886dc6f623a15c43"><xref rid="R133176324214622" ref-type="bibr">2</xref>, <xref rid="R133176324214624" ref-type="bibr">3</xref></sup> Endometriosis is a relatively common and potentially debilitating condition affecting up to 10-15% of reproductive age group women. It is seen in 30-40% of women with infertility. Certain guidelines given in <italic id="e-1ab2d9ea6daf">Samhitas</italic> are utilized for understanding such diseases.</p>
      <p id="paragraph-82af84118d094855a4a83e1a8f6e5d34">According to Ayurveda classics, endometriosis cannot be codified under a single disease entity. Our Acharya’s have emphasized <italic id="e-5d3dead0c9ae">Lakshanas</italic> (symptoms) like <italic id="e-6c5aa1bcfb0c">Shula </italic>(pain) throughout the <italic id="e-2cc2e36d00e0">Striroga Prakarana</italic> (Gynaecology), among which <italic id="e-8bac34fa3135">Vatiki yoni vyapad</italic>, <italic id="e-c0bf8df686e1">Udavartini yoni vyapad </italic>bear close resemblance to the clinical picture of endometriosis, which makes finding an effective, validated treatment protocol for the management of pain in endometriosis. <sup id="superscript-ddf84c83a3a6434b840a9d72b8595705"><xref id="xref-4b2db59d06da4c1da059329badee7b4b" rid="R133176324214620" ref-type="bibr">4</xref></sup></p>
      <p id="paragraph-d205e75c652f459a9a4f23187cad1e81">Acharya Charaka explains <italic id="e-849961ddbb99">Vatiki</italic> - a woman of <italic id="e-7712f5241c1a">Vata prakriti</italic>, when consuming improper diet and indulges in other activities capable of aggravating <italic id="e-4b0d0c95c4ad">Vata</italic>; thus, provoked <italic id="e-d6ec673342d5">Vata</italic> reaches the reproductive system and produces pricking and other pains, stiffness, sensation as if creeping of ants, roughness and numbness, local symptoms and fatigue or lethargy etc. Due to <italic id="e-1fce64455ec1">Vata Vaigunya </italic>menstruation appears with irregularity and is painful. <italic id="e-66b3b34115fe">Vata</italic> gets aggravated due to <italic id="e-ceed87c84769">Vega dharana </italic>and moves in the reverse direction, then settles in <italic id="e-53e2b34bbc90">yoni</italic> and produces the pain, initially pushes <italic id="e-ffb3f9d45261">raja </italic>in an upward direction, then discharges it with difficulty. The woman feels relief immediately after the discharge of menstrual blood in <italic id="e-c08e16cec557">Udavartini</italic>. <sup id="superscript-53a22a9d69844b098e58d3bf3415ad62"><xref id="xref-2a2515ceed8b476889ac631feb8d4729" rid="R133176324214621" ref-type="bibr">5</xref></sup></p>
      <p id="paragraph-b2e85e79b9d84cb3888665a4752d55fb">The Gynaecological reasons for chronic pelvic pain are diverse. This diversity ends in the clinical diagnosis of endometriosis in about one-third of the cases during their reproductive period. After a definitive diagnosis of endometriosis has been made out for those suffering from chronic pelvic pain, the patients are forced to travel through the traumatic paths of long-term medication leading to frustration and depression. This case study describes <italic id="e-d58142d0b023">Sodhana chikitsa</italic> and its effectiveness in the management of endometriosis.</p>
    </sec>
    <sec>
      <title id="t-eb7b55eae8e8">
        <bold id="strong-8c9541b4a3c44da3899284d54ed0b57b">Patient Information</bold>
      </title>
      <p id="paragraph-6921e6be0b034ee992ed0e77cc5e7c1c">A 37-year-old female (Swedish national) visited the Outpatient Department (OPD) of Ayurveda Hospital in December 2020 for the management of 7 years of chronic pelvic pain with chief complaints of severe lower abdominal and pelvic pain and heavy bleeding during menstruation. The patient reported that the pelvic pain starts one day prior to menstruation and stops 1-2 days after the onset of bleeding. The pain is most severe on the first day of the menstrual cycle, associated with heavy bleeding with clots. History of painful periods since menarche. </p>
      <p id="paragraph-ebe19701a181473687cff759f2306556">She was advised to take Oral Contraceptive pills (OC pills) in the year 2000, which she continued for 8 years. On medication, she got temporary relief from the symptoms, which reappeared on the stoppage of OC pills. She denied taking OC pills and resorted to oestrogen pills for the management of pain from 2008 to 2012. Stopped oestrogen pills when side effects- abnormal facial hair growth noted. From 2013 again, the pain and bleeding became worse. The pain aggravated, and she was advised for a USG scan in April 2015, which showed endometriosis, cyst in the left ovary. After which, she was advised by the Gynaecologist to have OC pills again. But she resorted to herbal medicine. She was unmarried but a non-virgin without having any habits or addictions. Her family history- Nothing specific revealed. Her personal history revealed a constipated bowel habit, occasional acidity and disturbed sleep. She attained menarche at the age of 12 years, a menstrual period with a duration of 3-4 days and an interval of 24-26 days. Her Last Menstrual Period (LMP) was on 03/11/2020. The amount of bleeding was excessive on the second day, evident from the usage of four to five heavily soaked pads. No history of hypertension, diabetes, thyrotoxosis, no surgical history was obtained. No history of any drug Allergy.</p>
    </sec>
    <sec>
      <title id="t-d5e7bbf6f241">
        <bold id="strong-989d295282564a79aa6f85bc57f47ebe">Clinical Findings</bold>
      </title>
      <p id="paragraph-6f64a359b3544a6680404b3120ae5e72">Patient was found to be average built and weighing 60.7 kg. Her blood pressure was 120/80 mmHg, pulse rate was 70/minutes, and BMI was 24.7 kg/m<sup id="superscript-dc87616f8c334c97bdc8c1d82fc3fbd3">2</sup>. <italic id="e-42363a513315">Ashta sthana pareeksha</italic> details are given in the diagnostic assessment. She is of <italic id="e-6fafd55c2bb1">Kapha-vata prakriti</italic> (~constitution based on bodily humour) with <italic id="e-5f387f3ec355">Madhyama satva</italic> (~mental constitution). <italic id="e-c1512fd4e786">Dasavidha rogi pareeksha</italic> details are mentioned in the diagnostic assessment. Examination of external genitalia didn’t reveal any abnormal findings. Per speculum examination showed a normal cervix without any significant abnormality. Bimanual examination revealed a bulky uterus; no tenderness was detected. Her bowel and bladder habits were normal.</p>
    </sec>
    <sec>
      <title id="t-8052eb29ee44">
        <bold id="strong-5860f7bb1a464931a30b4045dacb0d11">Diagnostic Assessments</bold>
      </title>
      <p id="paragraph-9bdba8de8072450aa825bccdc6fc2692">As part of initial assessment Ashta sthana pareeksha &amp; Dasavidha Pareeksha was done. <italic id="e-2d80fee94d73">Nadi</italic> (Pulse) was regular with 72 beats per minute, <italic id="e-bcc22a051d02">Mutram</italic> (Urine) was clear and flow and quantity was normal (Frequency of urination: 3-4 times during morning and 1-2 times at night), <italic id="e-bc14db0e8f39">Malam</italic> (Stool) was formed with nomal toilet once daily early morning, No coating was noted on <italic id="e-05abe8c48057">Jihva</italic> (Tongue) and the shape of tongue was normal, <italic id="e-aba9458666e9">Sabda</italic> (Sound) was normal, <italic id="e-f7b556a84126">Sparsa</italic> (Skin) was normal to touch, <italic id="e-29f69b391ab8">Drik</italic> (Vision) was normal. She was of young <italic id="e-2a87d86843b9">vaya</italic>: (age) and of moderate built and height <italic id="e-8fd949a7bda9">Akruthi</italic> (General appearance and built). Her (Basic constitution) <italic id="e-fe7473ce83b9">Prakruthi</italic> was assessed as <italic id="e-14ecb7064101">Kapha- Vata</italic>. <italic id="e-882fa2bd1fed">Vikruthi</italic> (Imbalances): <italic id="e-3fe701b95c9f">Vata, Kapha</italic> and <italic id="e-d7aa156dfed7">Pitta </italic>was assessed to be vitiated on <italic id="e-4a1dcab58fef">tridosha</italic> level, and <italic id="e-efedfed4d9f3">Dhushya</italic> (vitiation of tissues): <italic id="e-a2bb67094753">Dhatus</italic> (Tissue level) <italic id="e-18c7419b261f">Rasa</italic>, <italic id="e-bcd90deb25d5">Rakta</italic> and <italic id="e-63e267eefec7">Mamsa</italic> was found to be vitiated. (Quality and quantity of tissues) <italic id="e-d4bee8572741">Sara, Samhanana </italic>(Compactness), <italic id="e-d2aac8337b74">Satva</italic> (Mental strength and tolerance) were found to be normal, with moderate <italic id="e-798d56a67ac1">Vyayama sakthi </italic>(Exercise tolerance) and (Compatabilities) <italic id="e-0830721e2ac0">Satmya</italic>. </p>
      <p id="paragraph-cb79864ca3534ba09b065ffd1368c0b1">CBC-WNL, Urine –WNL Laboratory tests were unremarkable. Previous specialized evaluations included Radiological ultrasound revealed (abdominal ultrasonography), bulky uterus with adenomyosis. The left endometriotic cyst 5 X 2.3 cm was seen adherent to the uterus. A probable diagnosis, <italic id="e-85b98b2744df">Vatiki yonivyapat </italic>(~gynaecological conditions in Ayurveda which includes painful menstruation), was drawn out from the presenting complaints and the previous medical reports produced by the patient during the OPD visit on 11<sup id="superscript-9c6243f0137943a2849ead30c36e2599">th</sup> December 2020. She was advised for admission the same day.</p>
    </sec>
    <sec>
      <title id="t-825e93a4ee1b">
        <bold id="strong-dc4ae0061b9840fda0caa1b09e5bc514">Therapeutic Interventions</bold>
      </title>
      <p id="paragraph-afb74b57cd354b7ba296e9446f7d9380">The treatment given was <italic id="e-3c54ea55fd84">Amapachana</italic> (~digestion of undigested food), <italic id="e-a6abfe46d780">Agnideepana</italic> (~increase digestive fire), <italic id="e-f05831b0f5f4">Srotosodhaka</italic> (~clear channel) and <italic id="e-583eaff5b815">Vata anulomana</italic> (~downward movement of <italic id="e-26a9f8ece2ee">Vata</italic>), and finally <italic id="e-13c34660a508">Pitta </italic>pacifying. The therapeutic plan was to perform <italic id="e-71a410d4179b">Shodhana Karma</italic> (~cleansing therapies) after correcting her <italic id="e-7a3b4a6879a4">Agni </italic>(~digestive fire) by <italic id="e-e688a06c2cb9">Purvakarma</italic> (~preparatory therapies). Initially, <italic id="e-08081eb4a602">Hinguvachadi churna</italic> (6 g) was given for improving the digestive fire prior to cleansing therapy for three days.<bold id="strong-0fd7071509fc4381a443056d75a1cc07"> </bold>The sequence of treatment was as follows: </p>
      <sec>
        <title id="t-4fdcb41e2297"><bold id="strong-18b6036b0ba14dd7ba687a2ce3e79ad7">Preparatory therapy</bold> </title>
        <p id="paragraph-5bf4aeee76bf48aeb6ac85ca8b8a7ac1">The initial treatment was with <italic id="e-6a9d8178bbef">Agni vishamya</italic> (Aama) which is the main root cause of all diseases. Hence the medication given focused on <italic id="e-7fc23f1cc8b1">Agni deepana, Aama pachana, Vata anulomana</italic>. After that 15ml of <italic id="e-88a3053d63fa">Sapthasaram kwatha</italic> (Ref: Sahasrayogam) two times a day, followed by 6g of <italic id="e-0b470c9bf5ee">Hinguvachadi churna </italic>(Ref: Sahasrayogam) with warm water.</p>
        <p id="paragraph-b3140c00d64c4d1d9c28a7d8671ee723">Along with the above-mentioned treatment, 15 ml of <italic id="e-6fa0941c7203">Sukumaram kashayam</italic> (Ref: Ashtangahrudayam) two times a day, 15 ml of <italic id="e-f371c3f1d631">Thrayanthyadi kashayam</italic> (Ref: Ashtangahrudayam) two times a day, followed by 2 tablets of <italic id="e-55cfbee39fc8">Krimighna vati </italic>(Ref: Charakasamhita) daily, 30 ml of <italic id="e-37bc65678053">Asokarishtam</italic> (Ref: Sahasrayogam) after lunch and 10g of <italic id="e-99064e420d68">Manibadra gula </italic>(Ref: Sahasrayogam) at bedtime was given. The detailed schedule of treatment is tabulated in <xref id="x-e9ff7a47b439" rid="tw-31003f50995b" ref-type="table">Table 1</xref>.</p>

          <bold id="strong-101cedef7c964558be4f6bee1905e338">Asanas, Pranayama</bold>
        </title>
        <p id="paragraph-39ed6ceabdeb4ffdab0cc7c4774a09f6"><italic id="e-f58cb2ce6edc">Ushtrasana, Bhadrasana, Gomukhasana</italic>, and <italic id="e-7097fdd150df">Vajrasana</italic> were advised. <italic id="e-4acacc6f9528">Ustrasana</italic> also known as Camel Pose is a deep backward bending pose. <italic id="e-4a0d8802a302">Bhadrasna</italic> or the Gracious Pose is good for activating the <italic id="e-d019ca8d2bfd">Mooladhara chakra</italic>. In Sanskrit ‘Bhadra’ means ‘auspicious’ and ‘asana’ means ‘pose’. <italic id="e-e63dbab915e4">Gomukhasana</italic> A Sanskrit word, <italic id="e-fb6557d10c41">Gomukhasana</italic> literally translates into a cow face posture (go – cow, mukha – face, asana – pose). A seated yoga posture, <italic id="e-12bf3bfc5676">Gomukhasana</italic> can be performed along with a set of different seated Asanas. It helps stretch the arms, triceps, shoulders, and chest. <italic id="e-23a24b3dc54b">Vajrasana<bold id="strong-4d8b78a1584546948c5049fcce8b1191">-</bold>vajra</italic>’ means diamond-shaped or thunderbolt; ‘Asana’ means posture or pose. <italic id="e-0127fdc09f82">Vajrasana</italic> has been named after the shape it takes – a diamond or thunderbolt. </p>
        <p id="paragraph-58650a794d4248b09a83834ecb956866">Treatment was given for a duration of 42 days. Diet &amp; Behavioural changes advised. </p>
      </sec>
      <sec>
        <title id="t-870ddc356e57">
          <bold id="strong-ae55a905efa6479fa25ca145ebb8ab87">Follow-up and outcome</bold>
        </title>
        <p id="paragraph-9b11418ff9904163883b7716fb5846dc">The patient recovered significantly from pain lower abdomen; bleeding was normal. The menstrual cycle was regular and at normal intervals without any complications. After six months of follow-up, there have been no further episodes of pelvic pain and the assessment was repeated at six menstrual cycles (at 4<sup id="superscript-6afe167dc04a486392db09e335aa9b58">th</sup>, 5<sup id="superscript-ffcce94283d84cb99d081b0404844d4c">th</sup>, 6<sup id="superscript-b8d69624961e4888b50ed90545cfe9b8">th</sup>,7<sup id="superscript-ba2ff2d06c194e7388df8491f61fabe2">th</sup>, 8<sup id="superscript-965c5ec2c4cf40338e2df6a8a0931316">th</sup>, and 9<sup id="superscript-e3f95d0751744867acea6cd69d6933a5">th</sup> menstrual cycles without medicines).</p>
      </sec>
    </sec>
    <sec>
      <title id="title-121aea07dd4244eea694500cafe0619d">Discussion</title>
      <p id="paragraph-ee40c713f4874bacb9b792fe88fdd1fe">Endometriosis is one of the challenging clinical entities for the practitioners of Ayurveda. The complicated clinical scenario demands accurate diagnosis and management to prevent surgical intervention. This present case report emphasized the importance of an Ayurvedic treatment protocol for the management of pelvic pain connected with endometriosis. The patient was treated on the line of management of <italic id="e-ce7af8e63ec9">Vatiki yonivyapad</italic>. Treatment focused on <italic id="e-d66296f5a67e">Agni deepana, Aama pachana</italic> with <italic id="e-fa37d916d9a1">Shodhana</italic> in order to remove the <italic id="e-689f920b6b0f">Aama</italic> and get the <italic id="e-6bdd1a9da23c">Doshas</italic> back into normal. The treatment protocol aims for the pacification of <italic id="e-b78fabd8ed36">Vata dosha </italic>and correction of digestive fire, thereby abolishing the chronic pelvic pain associated with endometriosis. Due to the unavoidable role of <italic id="e-7c472efc4b1e">Vata dosha</italic> in the present disease entity, cleansing therapies like purgation and enema procedures hold prime importance for the pacification of <italic id="e-7861f5cec6d7">Vata dosha</italic>. </p>
      <p id="paragraph-c8bd63e1bc5441db8b14877e5fdb0280">A course of medicated enema with intrauterine instillation of medicated formulation has been selected after purgation, giving due importance to the analgesic properties of the drugs. Same time Pitta pacifying medicines were given at the time of discharge. Follow up USG was advised, which was politely refused by the patient. However, she was constantly in touch with proper follow up of oral medicines. For the last two years, no follow up possibly due to a reduction of symptoms.</p>
      <p id="paragraph-6678131cad93482ebbd7b344ad2bfc6c">Previous studies have proved the efficacy of <italic id="e-492536abacb4">Sodhana </italic>and <italic id="e-a592409a71e0">Samana</italic> therapy in the management of chronic pelvic pain in endometriosis <sup id="superscript-37e32eac561a49e0aa5433186d4927f3"><xref id="xref-8f8f7c9f99e54142962bce6b6bb296b8" rid="R133176324214625" ref-type="bibr">6</xref></sup> and infertility as a consequence of endometriosis.<sup id="superscript-efbcd304b84f4f08b5aa8b18ae262285"> </sup><xref id="xref-7246d4bd813448e4bf704edb29d3b2f8" rid="R133176324214626" ref-type="bibr">7</xref><sup id="superscript-68ea9eb76e394efba7e5fb9249d2a3e7"> </sup>This study results were also in agreement with the previously reported studies. When the treatment principles are individualized and applied clinically, it yields excellent results to patient satisfaction. Ayurveda blesses the feminine world with numerous drugs, which helps to maintain their health in a natural way. Hence Ayurveda plays a major role in treating endometriosis and thereby prevent complications.</p>
    </sec>
    <sec>
      <title id="title-1908f6e9a44a4f84a00ffe32d6af5813">Conclusion</title>
      <p id="paragraph-f3a9cab4ed8c4898920a9fdc84d104bc">This case study showed that if the plan of treatment is selected according to principles of Ayurveda along with proper drug, dose, duration, <italic id="e-151580370216">Anupana, Pathya</italic> and <italic id="e-57ac109e3f15">Apathya</italic> there is an assertion of progress in treatment. The Ayurvedic management protocol - <italic id="e-0d0254b66fb7">Sodhana chikitsa </italic>is highly effective and recommended in managing the chronic pelvic pain associated with endometriosis. </p>
      <sec>
